The contents list below is very much part of the work in progress and the structure will undoubtedly change as time goes by. It is however a pretty good indication of what's going to be in the book, although the sequencing might be different from what you see here. The contents of the bibliography as it presently exists has been included in case anyone thinks that I've missed out any potentially useful sources.
